Counseling Forms for Students

All students will need their MyGWC Username and Password in order to fill out the form correctly.

Early Grade Report

If you are not passing a course in the current semester and intend to register for the same course in the following semester in order to make up the grade, use the Early Grade Report to allow Enrollment Services to “force” the drop of the class, allowing you to register for the same course in the upcoming semester.

Overload Petition

Use the Overload Petition to request to enroll in more than the maximum number of units per term allowed.  You will need the Add Code(s) for the courses you intend to enroll in over the maximum. See form for additional GPA requirements.

  • Fall – 19 units maximum
  • Intersession – 6 units maximum
  • Spring – 19 units maximum
  • Summer – 9 units maximum

Petition for Degree/Certificate Course Requirement Substitution

If you believe you have met a requirement for a degree or certificate offered at GWC at another community college or university, use have your counselor submit the Petition for Degree/Certificate Course Substitution form.

Prerequisite Clearance

If you received a Prerequisite and Test Score Error during registration, please check the prerequisites by clicking on the CRN of your chosen course from the Class Schedule. These courses must be completed prior to enrolling in your selected course.

If you believe you have met the prerequisites for the course with a course completed at another regionally accredited college, you may submit a prerequisite clearance request. Please see below.

SUBMISSION REQUIREMENTS

Prerequisite Clearance Request must be submitted at least 10 days prior to your registration appointment. Please allow 5 to 7 business days for processing. Please upload any necessary unofficial documents (transcripts, AP scores, etc.) with this request.

If your official transcripts are already on file with the Admissions and Records Office, you will not need to upload additional unofficial documents.

Prerequisite Clearance Form

Prerequisite Clearance: 1st 8 weeks to 2nd 8 weeks

If you are taking a course during the 1st 8-weeks of the semester that is a prerequisite for a course you intend to register for during the 2nd 8-weeks, use this form to show proof of enrollment, allowing you to register for your next course.

EXAMPLE:
Enrolled in ENGL G100 during the 1st 8-weeks and intending to register for ENGL G110 during the 2nd 8-weeks

Prerequisite Clearance: Winter Intersession to Spring

If you are intending to register for a course during the Winter Intersession that is a prerequisite for a course you intend to register for during Spring semester, use this form to show proof of enrollment, allowing you to register for your next course.

EXAMPLE:
Enrolling MATH G160 (Intro to Statistics) during the Winter Intersession and intending to register for PSYC G280 (Research Methods) in the Spring semester.

Repeated Course Grade Petition

Use the Repeated Course Grade Petition when repeating a course in which the grade received was a “D”, “F”, or “NP” and you would like to have the original grade excluded and not calculated into your GPA.

Transcript Evaluation

If you have had official transcripts sent to Golden West College, you may request an official transcript evaluation to see how your previous coursework may apply toward your studies at GWC.  Please allow two to four weeks for your evaluation to be completed. You will receive a notification once your evaluation is complete.   

Students may also make an appointment with a Counselor and request an official evaluation for the meeting.  This is more beneficial as you can work with a Counselor to apply your previous credit as you develop your Student Education Plan (SEP). Please contact Counseling and request an appointment for Transcript Evaluation.
